  • Essential Job Functions: At Children’s Continuum of Care, our mission is to provide “Behavioral In-Home Care continually transforming lives”. CCoC provides behavioral health services to children and families referred through The Department of Children and Families, Children’s System of Care and the Division of Child Protection and Permanency throughout New Jersey. Interns in our program will gain a greater understanding of their own social identity, through exposure to the social identity of the children, youth, young adults and families that CCoC serves. Interns will have the opportunity to collaborate with Therapists, Behavioral Assistants, and the Administrative Team through case presentations, shadowing in home visits, and various trainings. CPC Behavioral Healthcare

  • Essential Job Functions: This is a temporary internship position. Internships are offered to students enrolled in an accredited program at a college or university where the internship is a required component of the academic curriculum. Internships are offered for Bachelors and Masters level students of Social Work, Counseling and related programs. Interns are an integral part of the multidisciplinary treatment team where they are assigned. Under the supervision of a CPC field supervisor, first semester masters level practicum interns observe and co-facilitate treatment services for individuals that may include individual, group and/or family sessions as well as coordination of case management and psychiatric services as indicated on the treatment plan. After the practicum semester, which may have occurred at a different agency, masters level interns may additionally meet individually with clients. Responsibilities and duties will vary depending on the student’s enrolled program and stage of their studies/course completions
    • Attend 2 hour New Intern Orientation session as scheduled;
    • Complete additional training modules within the agency’s electronic learning management system as required by NJ licensing standards and agency policies – trainings are asynchronous and may need to be completed outside of regularly scheduled internship hours;
    • Meet with your assigned supervisor at least once per week;
    • Assess the counseling needs of the clients;
    • Assess clients using standardized clinical interview tools, and diagnosing clients for mental health and or substance use disorders using the DSM-5;
    • Determine the appropriate level of care for substance use treatment (if applicable) according to ASAM Criteria;
    • Obtain previous records that are relevant to the current treatment episode;
    • Unless clinically contraindicated, collaborate with the client to develop a written treatment plan that includes goals and measurable objectives, and is client centered, recovery oriented;
    • Provide mental health and/or substance abuse counseling services specified in the client treatment plan;
    • Evaluating clients throughout the treatment episode to determine the need for continued services, transfer, or discharge;
    • Review and, where necessary, revise the client treatment plan to address ongoing problems;
    • Develop a client discharge/transfer plan to ensure movement to the appropriate levels of care;
    • Contact, case consultation if necessary, and coordination with referral sources, (for example, mental health treatment providers, criminal justice agencies, schools, employers, DCP&P);
    • Participate as a member of a multidisciplinary team for assigned clients;
    • Document all counseling and education services, assessments, reassessments, referrals and follow-up in the client’s clinical record;
    • Complete Biopsychosocial Assessment within the time frame established for the program.
    • Follow Agency and regulatory policies regarding confidentiality and security of client information.
    • Abide by Agency, State and Federal as well as professional licensing standards pertaining to client rights, ethical treatment, conflict of interest, HIPAA, etc.
    • Identify High Risk Clients and work with team to provide safe and effective treatment outcomes.
    • Report incidents, client complaints, serious concerns regarding risk levels, etc. to supervisor. Utilize Agency policy and procedures to address these issues.

  • Additional Details:
    • Interns will need their own personal laptop computer to use during a field placement at CPC Integrated Health. No equipment is provided. Interns are provided with credentials at the start of their field placement to access agency email, electronic health record system and learning management system.
    • The length of internships is determined by the assigned CPC supervisor and requirements of the college/university. While rare, CPC reserves the right to dismiss interns for failure to uphold responsibilities and duties, failure to communicate with staff, failure to satisfactorily complete work, unprofessionalism, or neglecting any duties.

            • Mumps, Measles and Rubella Titer
              • Holisticare does cover the MMR Titer test free of charge if you go through us.
                We do not cover the cost, if you choose to go through your own doctor.
              • If you have already received the MMR vaccine within your lifetime previously, we will count that, but we would need a copy of your records.
            • COVID 19 Vaccination
              • While Holisticare Hospice does not require volunteers to be COVID 19 Vaccinated, in order to volunteer the State of New Jersey does require that all members who have gotten the initial Covid 19 vaccination (whether it be the 1 or 2 step series) be boosted at least once. We do not provide or cover the cost of the Covid 19 Vaccination and or Booster.
              • For those who do not plan of getting Covid 19 Vaccinated or boosted at least once – I will provide you with a copy of our COVID 19 Medical and or Religious Form to complete.
              • For those who are COVID 19 Vaccinated and have received at least 1 booster – I will collect your card towards the end of your onboarding process.
              • For those who have been vaccinated with the initial series but have not had at least one booster- You can either elect to get the booster, or I will provide you with a copy of our COVID 19 Medical and or Religious Form to complete.
            • Influenza Vaccination
              • The State of New Jersey requires that health care workers (which includes volunteers) to be vaccinated against the flu, during flu season every year.
              • For those who have received the Flu Vaccine- I will collect your card towards the end of your onboarding process.
              • For those who want to receive the Flu Vaccine – We do not provide or cover the cost of the Flu Vaccination. You are able to get it done free of charge through your health insurance at many local pharmacies and will need to do so prior to when you begin volunteering.
              • For those who do not plan of getting Flu Vaccinated – I will provide you with a copy of our Flu Medical Form to complete. The State of NJ does not accept Medical Exemption Forms.
          • Non-Mandatory Medical Tasks
            • Hepatitis B Vaccination
              • Holisticare Hospice does not require volunteers to be Hepatitis B Vaccinated, in order to volunteer.
              • For those who have received the Hep B Vaccine within their lifetime – We will count that, but we would need a copy of your records.
              • For those who have not received the Hep B Vaccine within their lifetime – Holisticare does cover the Hep B test free of charge if you go through us. We do not cover the cost, if you choose to go through your own doctor.
              • Please note – It is a 3 series vaccination, that requires you to get it done initially, then go back to get the second shot a month after and receive the last shot 6 months later

              • If you choose not to get it done – I will provide you with a Hep B Declination Vaccination Form to sign off on which will suffice.
      • Step 5: Finalized Training
        • We will then connect in person to review some additional training to finalize everything. We will discuss how you will be paired with patients, the forms that you will receive, along with documenting your visiting online, dress code, etc. I will also take your photo for your volunteer badge and collect any other forms that are needed to complete your volunteer file.
